Day Habilitation

Transitional Living Services

Transitional Living Services are designed to provide services to individuals with severe and persistent mental illness, intellectual disability, or brain injury who are leaving care to begin community living or are living in the community and need services to help them remain in the community successfully. Individuals with a diagnosis of severe and persistent mental illness or co-occurring disorders are eligible to receive services. Services are provided to the clients in their homes in a natural setting. The clients are provided opportunities to maintain, enhance, or develop skills in a variety of areas to promote and support independent living.

Transitional Living Services can be provided under Habilitation Services or HCBS Waiver Services or other funding sources as appropriate to each individuals circumstances. This can be accomplished with hourly services in the persons own housing/apartment, supervised apartments on the Penn Center, Inc. campus with on-site services/support 10-12 hours per day, Cedar Rapids site with staff available up to 16 hours per day or can be provided in a daily 24 hour home based habilitation setting with staff on site in the home 24 hours per day. Crisis intervention is provided on a 24 hours per day, 7 days a week basis.

Transitional Living Services provides opportunities for individuals to work towards and achieve personal and healthcare related goals, facilitates a safe environment for individuals to flourish while providing opportunities to fully integrate into the community in which they have chosen to live. Special emphasis is placed on empowering individuals to take charge of their own recovery process by learning the skills to manage their illnesses, live independently and develop natural supports.

Referral Process:

  • Referrals are accepted by self referral, family referral, case management referral, or other healthcare provider such as family physician or hospital

Admission Process

  • Desire for individual to work with Transitional Living staff toward personal goals while living in the community
  • Funding source to cover the service costs
  • Identified case manager to assist with service needs
